Fees

• 4%= of gross collections after discount for charity match and CE credit (Others are 3% for name and no support, and 2% for MLM fee to founding office). FDD pg 5; CFDFA 14-15
• 2% of gross collections for General Marketing Fund (others are also 2%). FDD pg 6; CFDFA pg 23
• 4% of gross collections for Maintenance Fund/ "Emergency Fund". (Fund to be collected over $1,000 floor only if each full time partner is taking home at least $13,000 net pay in any given month. Funds kept separate and only to be used for respective office).FDD pg 6; CFDFA pg 27
• 10% fee when ownership is transferred. FDD pg 7
• Office sublease from Robert Baird LLC. This is how you fund the Humanitarian Match Fund. FDD pg 21
• 1% royalty discount/match given if a doctor does 3% of his collections in charity per year. Coupon offers do not count ($300*32hr*50 weeks*3%=$14,400 a year or $1,200 a month). CFDFA pg 15
• 1% royalty discount given if doctor does 30 hrs of CE per year or 2x Utah requirements. CFDFA pg 15

• Cascade Contract Services: these services are billed as a separate expense. In the management of such services, the owner can not make more then a 20% administration fee. At the end of each year, offices can request a summary report of related revenues and expenses. Numbers are a percentage of their collections.
• No Franchise Renewal Fee for 2nd term.
