Set complete the proper procedure codes

1 Person 1 year           MemPlan 1         code:01

2-3 People 1 year             MemPlan2-3      code:02

3-4 People 1 year             MemPlan3-4      code:03

5-6 People 1 year             Memplan5-6      code:04

7+ People 1 year              Memplan 7+      code:05

Notes

• Change Provider to "Shared Hygiene" in account module.
• Two options for payment: Re-occurring payments (monthly) or Paid in full.
• If this is the first time the family has signed up for the Membership Discount Plan, you MUST set complete the Set Up Fee of $60 (unless otherwise noted by a doctor)
• Follow a typical payment plan set up with the first payment being due TODAY.

Example

• Membership plan cost: $288 + Set up Fee of $60 + Monthly Billing Charge of $48 = TOTAL: $ 396

• Divide that Total by # of months (12) = $33 a month for payments. The first payment is due today.
