• Card may say Pinnacle or Summit (Both OK)
• Pinnacle plans can use both in and out of network providers
• Summit plans can only use in-network providers
• Always use the Dentist Direct Fee schedule! Dentist Direct also uses Dentemax fee schedules; however, since we are providers of both they should always be using the Dentist Direct.
• Will always have a Member # and Group #
