• 2 deductibles apply: $50 for basic AND $100 for major services. Non for basic.
• This is the only plan that patients can switch too at any time and not have to wait for their open season.
• $1,200. Yearly max
• 1 year waiting period on all Major services (Perio, Endo, and Oral surgery fall under major.  Exclusion = Simple extraction codes: D7140, D7210, & D7250 fall under basic and don’t apply to waiting period)
• 2 year waiting period applies to orthodontics
• Missing tooth clause applies
